A recent meta-analysis undertaken for the National Asthma Campaign in preparation for the revised Asthma Management Handbook showed that combination therapy with an inhaled corticosteroid and a long-acting beta agonist achieved statistically greater improvements in lung function tests than inhaled corticosteroids alone in patients aged 4-80 years who had previously not received corticosteroids. These improvements may not always be of clinical importance, but combination therapy also resulted in fewer exacerbations in patients who were symptomatic on inhaled corticosteroids alone.

Advair (Fluticasone + Salmeterol) :    The original combination inhaler is still the most popular.     Basically it combines a steroid with a long acting beta adrenergic (LABA).   The steroid  controls inflammation while the LABA keeps your lungs relaxed around the clock.   The combination of these medicines has worked wonders for many asthmatics.  The best part of this medicine is all you need to do is take one puff in the morning and one before bed.   This greatly improves complaince, and that alone I think has benefited many asthmatics.

Nebulisers are machines that turn the liquid form of your short-acting bronchodilator medicines into a fine mist, like an aerosol. You breathe this in with a face mask or a mouthpiece. Nebulisers are no more effective than normal inhalers. However, they are extremely useful in people who are very tired (fatigued) with their breathing, or in people who are very breathless. Nebulisers are used mainly in hospital for severe attacks of asthma when large doses of inhaled medicines are needed. They are used less commonly than in the past, as modern spacer devices are usually just as good as nebulisers for giving large doses of inhaled medicines. You do not need any co-ordination to use a nebuliser - you just breathe in and out, and you will breathe in the medicine.
